Antibiotic Regimen

Piperacillin-tazobactam 4.5 g IV q6h x 7 days

IF MRSA RISK FACTORS PRESENT, ADD

Vancomycin

Beta-lactam Anaphylaxis

Alternative for patients with true beta-lactam allergy only. Refer to beta-lactam allergy assessment algorithm.

Moxifloxacin 400 mg PO* q24h

x 7 days

*If PO is not feasible, then IV is acceptable in these situations.

OR

Meropenem* 500 mg IV q6h

x 7 days

If *Pseudomonas risk is high

Note

If confirmed Pseudomonas pneumonia, duration of therapy is 14 days.
