Penicillins and Carbapenems

Cross-reactivity between penicillins and carbapenems is low (~1%)

Recommendations for Management

In patients who do NOT have evidence of an immediate (type 1) allergic reaction to a penicillin (see below for definition), any carbapenem antibiotic can be used.

In patients who have a history of an immediate (type 1) allergic reaction to a penicillin, carbapenems may be used with caution.

If there is no suitable alternative antibiotic and a carbapenem is deemed necessary, the carbapenem may be used following successful desensitization.
