Severely Ill /immunocompromised

Treatment of patients with perforation of esophagus, stomach, small bowel, or colon.

Zosyn 3.375 g IV every 8 hr (4 hr infusion)

PCN Allergy

Tobramycin 5 mg/kg IV every 24 hr

PLUS

Metronidazole 500 mg IV every 8 hr

Duration

5-7 days, may require longer
