Antimicrobials
Can be a contaminate. Draw repeat cultures prior to starting therapy to confirm infection.
In 2017, 98% of E. faecalis isolates at Memorial were susceptible to ampicillin, which should be used first unless the patient has an allergy to PCN.
Consider echocardiagram to rule out endocarditis if there is persistent bacteremia >3 days on appropriate antibiotics.
Do not use gentamicin for synergy if the lab lab reports no synergy with gentamicin; doing so will only increase the risk of nephrotoxicity without clinical benefit.
Ampicillin 2 g IV every 4 hr
Vancomycin
10-14 days
