Patient requires ICU admission

Hospital admission >5d or has been on antibiotics in past 90d, or structural lung disease with frequent antibiotic use (e.g. COPD)

Significantly immunocompromised or significant travel history

Significantly immunocompromised: Solid organ or bone marrow transplant;

Active leukemia or lymphoma;

receiving chemo for malignant or inflammatory conditions;

Receiving systemic steroid (prednisone >25 mg/day)

Colonized with resistant pathogen known to cause pneumonia
