Many orthopedic procedures have P&T -approved order sets. In these cases prophylaxis as per order sets is also acceptable.

Procedure Type

Minor procedures
  •  Clean operations involving hand, knee, or foot and not involving implantation of foreign materials (If the potential for implantation of foreign materials is undetermined, the procedure should be treated as with implantation)

  • Spinal procedures with and without instrumentation

  • Hip fracture repair

  • Implantation of internal fixation device (e.g., nails, screws, plates, wires)

  • Total joint replacement
