Most common cause is probably the administration of antibiotics prior to adequate blood culture collection, especially when streptococcal

Fastidious organisms: Coxiella, Bartonella, Brucella, T. whipplei, Legionella, and others

Serologic testing can be considered in the context of the history (travel, activities, animal contact)
