S.aureus CRBSI

Remove line - catheter retention strongly discouraged

Continue vancomycin until susceptibilities available; if MSSA narrow therapy to cloxacillin 2g IV q4h or cefazolin 2g IV q8h

Repeat blood cultures q24-48h until negative

Duration of therapy (2-6 weeks) depends on presence of complications (e.g. endocarditis) and persistence of bacteremia- see algorithm and discussion on S. aureus bacteremia duration of therapy.

If line retention necessary (ie. sole vascular access, line required for life-sustaining therapy) , consultation with Infectious Diseases recommended.
