Always collect blood culture for pyelonephritis in long term care
Review previous urine culture data if recurrent UTI
IV: Ceftriaxone 1g IV q24h
OR
PO: Ciprofloxacin 500 mg PO BID
When to use empiric fluoroquinolones:
Mild to moderate illness only
Tolerating oral intake
When likelihood of E.coli resistance is low (minimal antibiotic exposure)
