Antimicrobials
Hemodynamic instability, altered LOC, hypoxemia, new infiltrate on CXR, GI symptom, catheter tunnel site infection, OR cellulitis >5cm
Anticipated duration of neutropenia >7 days
Hematologic malignancy, especially AML or allo-HSCT
Previous fungal infection
Significant symptoms, i.e. dyspnea, pain
Oral mucositis with ulceration that affects eating (or worse)
Significant comorbidity, including uncontrolled COPD, renal failure with eGFR<30, hepatic dysfunction
Colonized/previous infection with a resistant organism
