Patients with synovial fluid crystals may still have septic arthritis (1.5%)³
Progressively higher synovial WBC count increases the likelihood ratio (LR) of septic arthritis:
Synovial WBC count >50,000/microL has a + LR of 7.7 for infection⁴
Percentage of neutrophils >90% has a = LR of 3.4 for infection (for any WBC count)⁴ ⁵
