MRSA Monitoring

Early infectious diseases consultation is strongly recommended

Trough vancomycin level should be drawn 30 minutes prior to the 4th dose or consult with clinical pharmacist

If Vancomycin MIC greater than 1.5 mg/L, consult Infectious Diseases for alternative treatment options

Repeat blood cultures are required to determine clearance of bacteremia. Order repeat blood cultures on day 2, then assess the need for repeat blood cultures Q2 days until clearance

Please DO NOT write routine orders for daily blood cultures

Persistent bacteremia from blood cultures obtained 4 days after initiation of antibiotics suggests inadequate source control or warrants reassessment of current antibiotic therapy
