Vancomycin loading dose of 25mg/kg IV x 1 dose followed by 15mg/kg IV Q12H
View vancomycin nomogram
Reassess daily for PO stepdown. A positive response includes: resolution of fever, no rapid progression of affected area, improved local pain
Consider Infectious Disease Consult if failure to improve after 3 days
