Initial Empiric Treatment

Add metronidazole to either cephalosporins or levofloxacin to include coverage for obligate anaerobic bacteria.

The use of clindamycin is discouraged due to added risk of C. difficile infection when combined with other broad-spectrum agents

Piperacillin-tazobactam and carbapenems provide adequate anaerobic coverage if selected for empirical therapy of P. aeruginosa or MDR gram-negative bacteria, respectively.
