Native valve coagulase negative Staphylococcal endocarditis does occur but is uncommon.
It is more likely in the setting of underlying valve pathology.
4-6 weeks from first negative blood culture.
If valve replacement required, therapy may need to be altered to reflect prosthetic valve endocarditis.
Vancomycin 25-30mg/kg IV load (max 2g) followed by 15mg/kg IV q12h maintenance for a target trough of 15-20µg/mL
